Letter of Introduction
Donna Diotalevi, RN CCM CRRN
Thank you for your interest in my services. As a Professional Care Manager, I assist individuals with all aspects of their lives. My goal is to assist older adults and their families, meet the challenges of aging, by focusing on capabilities and desires rather than limitations. I collaborate with a network of other experts to facilitate the best quality of life for my clients.
I have been a Registered Nurse in the Tampa Bay area for over 30 years. I was one of the first professionals to obtain National Certification as a Case Manager (CCM) in 1993. In 2000 I also obtained National Certification as a Rehabilitation Registered Nurse (CRRN). I completed the Florida Assisted Living Administrators (CORE) training program, followed by certification through The Florida Department of Elder Affairs in 2006.
My services are geared towards serving the needs of seniors and their families, but I also provide case management services for those with disabilities, chronic diseases (like diabetes, COPD, heart disease and arthritis) and catastrophic diagnoses (such as cancer, stroke, brain and/or spinal cord injuries).
I am an active member of the National Association of Professional Geriatric Care Managers (NAPGCM), as well as the Florida Geriatric Care Managers Association (FGCMA) and the American Holistic Nurses Association. I am committed to maximizing the independence and autonomy of older adults and people with disabilities, while striving to ensure that the highest quality and most cost-effective health and human services are used when and where appropriate.
My services generally begin with a telephone consultation, followed by a visit to the care recipient and their family, to complete a comprehensive assessment of medical, social, financial, and legal concerns.
Following the comprehensive assessment, problems and goals are identified and a care plan is developed. With client approval, recommended services are coordinated and ongoing care needs are monitored.
The total cost of Case Management services will depend on the frequency and intensity of intervention that is requested. Sometimes just a little assistance can allow a loved to remain in their home but if other living arrangements are needed, I can assist with alternative arrangements, such as selection of, and transition to, an assisted living facility (ALF). I am knowledgeable of a wide variety of community resources and support options that can help make life easier.
Care Managers save money and reduce family burden by coordinating appropriate care and resources and avoiding unnecessary expenditures. One of my many goals is avoidance of crisis, unnecessary hospitalizations and inappropriate placements. Care Managers can also reduce costs by negotiating discounts for medical services and supplies.
