 Committed to maximizing the independence and autonomy of older
adults and those with disabilities, while striving to ensure that the highest quality and most cost-effective health and human
services are used, when and where appropriate.
PROFESSIONAL LICENSE AND CERTIFICATIONS:
Donna
Diotalevi has been a Registered Nurse for over 30 years. She obtained National Certification as a Case Manager (CCM) in 1993.
In 2000 she obtained National Certification as a Rehabilitation Registered Nurse (CRRN). These certifications require sitting
for and passing a National exam, after demonstrating work experience under another certified professional. Prior to entering
the field of case management in 1990, Donna spent the first half of her career in the mental health field and was employed
at the USF Psychiatry Center and the Florida Mental Health Institute. Donna is involved in continuing education to the
community on elder-care and disability issues.
She particularly enjoys working with those
suffering from mental health issues and/or memory disorders. She facilitates a support group through the Florida Gulf Coast
Chapter of The Alzheimer's Association.

Though services are geared towards serving
the needs of seniors and their families, care management services are also provided for those with disabilities, chronic diseases,
catastrophic diagnoses and work related injuries.
